padding: 0px 0px 25px; color: rgb(64, 64, 64); font-family: 'Helvetica Neue', Helvetica, Arial, sans-serif; font-size: 14px; line-height: 25.2px; background-color: rgb(255, 255, 255);">首先，可能导致严重肝损害。从单纯性脂肪肝进一步发展为脂肪性肝炎、肝纤维化、肝硬化，死亡的风险大大增加。其次，脂肪肝可以加重胰岛素抵抗，加重糖代谢紊乱，尤其是到重度脂肪肝或肝硬化时，由于肝功能异常，不能将血液中过多的葡萄糖转化为肝糖原储存，导致血糖难以控制，加重糖尿病病情。</p><p style="margin: 0px; padding: 0px 0px 25px; color: rgb(64, 64, 64); font-family: 'Helvetica Neue', Helvetica, Arial, sans-serif; font-size: 14px; line-height: 25.2px; background-color: rgb(255, 255, 255);">同时应告诫患者每日坚持进行中等量的有氧运动，通过生活方式干预，积极控制体重。体重下降 3%-5% 可以有效改善肝脏脂肪堆积，体重下降 10% 可以有效减轻肝脏炎症。</p><p style="margin: 0px; padding: 0px 0px 25px; color: rgb(64, 64, 64); font-family: 'Helvetica Neue', Helvetica, Arial, sans-serif; font-size: 14px; line-height: 25.2px; background-color: rgb(255, 255, 255);">2. 积极治疗糖尿病，严格控制血糖</p><p style="margin: 0px; padding: 0px 0px 25px; color: rgb(64, 64, 64); font-family: 'Helvetica Neue', Helvetica, Arial, sans-serif; font-size: 14px; line-height: 25.2px; background-color: rgb(255, 255, 255);">良好的血糖控制有助于肝内脂肪浸润消退，应根据患者的实际情况制定个体化的血糖控制目标。在选择降糖药物时，宜选择胰岛素增敏剂，如双胍类和噻唑烷二酮类。二甲双胍能有效改善胰岛素抵抗、控制体重。比格列酮不仅有助于改善胰岛素抵抗，而且能有效改善肝脏脂肪堆积和炎症。若患者转氨酶超过正常上限的 3 倍，应考虑换用胰岛素治疗，以避免增加肝脏的负担。</p><p style="margin: 0px; padding: 0px 0px 25px; color: rgb(64, 64, 64); font-family: 'Helvetica Neue', Helvetica, Arial, sans-serif; font-size: 14px; line-height: 25.2px; background-color: rgb(255, 255, 255);">3. 药物治疗</p><p style="margin: 0px; padding: 0px 0px 25px; color: rgb(64, 64, 64); font-family: 'Helvetica Neue', Helvetica, Arial, sans-serif; font-size: 14px; line-height: 25.2px; background-color: rgb(255, 255, 255);">保肝药物种类很多，应当选择安全性好、疗效确切的保肝药物，如多烯磷脂酰胆碱、水飞蓟素等，上述药物可通过修复肝细胞膜、减少氧化应激损害等机制保护肝功能。 虽然氧化应激是脂肪性肝炎病情进展的重要机制机制机制，糖尿病合并脂肪性肝炎患者，并不推荐应用维生素 E 治疗。</p><p style="margin: 0px; padding: 0px 0px 25px; color: rgb(64, 64, 64); font-family: 'Helvetica Neue', Helvetica, Arial, sans-serif; font-size: 14px; line-height: 25.2px; background-color: rgb(255, 255, 255);">（2）降脂药物： 目前认为，脂肪肝如果不伴有高脂血症，不必用降血脂药物，因为此类药物并不能有效改善肝脏病变。</p><p style="margin: 0px; display: block; width: 232px; height: 183px;"></div><div class="m-content" style="line-height: 20.8128px;">1992 年至今，樊嘉院长已经在中山医院工作了 24 年。说到中山精神，他语气里充满自豪。他说：「中山医院建院已近 80 周年，八十年来，医院发生了巨大的变化，但不管怎样变化，从老一辈创院伊始到现在，学风严谨、对待患者无私奉献、一切想着患者的优良传统代代相传，一切为了患者的中山精神始终未变。」</div><div class="m-content" style="line-height: 20.8128px;">&nbsp;</div><div class="m-content" style="line-height: 20.8128px;">樊嘉院长概括中山精神包括三个层面：第一，对患者的态度要好，要同情患者，多安慰，设身处地地为患者着想；第二，要致力于解决患者的疾病和痛苦，这是最重要的，要求医生有严谨的学风、扎实的技术和本领；第三，要有极强的社会责任感。作为公立医院，在重大疫情和突发状况如地震、海啸、传染病等面前，要不计个人得失和安危地走在前面。这三个层面综合起来才是中山精神的完整体现。中山人，不管是管理者、专家和广大医务人员，还是后勤人员，都是围绕这三个层面并以其为核心来服务患者、提高医术、不断升华医德医风，从而更好地为患者、社会、国家服务。</div><div class="m-content" style="line-height: 20.8128px;">&nbsp;</div><div class="m-content" style="line-height: 20.8128px;">在中山医院工作了 24 年的樊嘉院长身体力行地实践着中山精神。作为肝脏外科医生，高强度、高风险几乎是樊嘉院长的工作标签。繁杂的医疗和管理工作使得他长期保持每周工作 100 个小时，大量肝癌手术大大增加了他接触乙肝病毒、丙肝病毒的几率，甚至经常要面对手术污血溅入眼中的风险。正是因为一切为了患者的中山精神，樊嘉院长坚守在肝脏外科第一线，乐此不疲。他常说：「我们应该再给生命一次机会。」为此，他不断挑战手术难度，尝试手术禁区。至今，樊嘉院长已主刀完成了 8000 余例肝肿瘤切除手术，他首创的肝癌门静脉癌栓多模式综合治疗技术解决了门静脉癌栓形成这一影响肝癌治疗的世界性难题，使伴门静脉癌栓的晚期肝癌由不可治变为部分可治，患者生存率显著提高，并被写入原卫生部原发性肝癌诊疗规范。在肝移植这一肝外科的尖端领域，樊嘉院长同样成绩骄人，提出的我国肝癌肝移植适应证—「上海复旦标准」获得广泛认同。已成功实施各式肝移植 1500 多例，其中包括亚洲首例成人肝心联合移植、上海市首例成人—成人右半肝活体肝移植、上海市第一、二例成人—儿童活体肝移植、中国首例经典劈裂式肝移植、国内年龄最小的劈裂式肝移植、国际年龄最大的受体肝移植等，不断创造着「世界奇迹」。</div><div class="mt30" style="margin-top: 30px;
